Catholic Health East is committed to providing person-centered, compassionate, holistic care. In an effort to accomplish this goal, we are embarking on a system-wide clinical transformation initiative called CareLink.
CareLink is an evolving clinical transformation initiative that will leverage technology to integrate advanced clinical information, evidence-based practices, decision support and other clinically based tools to provide high-quality, comprehensive, efficient and person-centered care. The CareLink initiative is an evidence-based model incorporating order sets, interdisciplinary plans of care and computerized provider order entry. CareLink supports clinical workflow and will be available to providers and clinicians at the point of care to enhance treatment, outcomes, proficiency and care coordination.
Transforming service delivery with evidence-based care and best practices
Evidence-based care is the conscientious, explicit and judicious use of current best evidence in making decisions about patient care. Leveraging technology, CareLink provides the model to establish an electronic hospital-wide resource for order sets, leading clinical practices, and information to enhance care, patient safety and coordination of services. Physicians and colleagues will have access to this information, allowing timely decision-making and consistency of care.
CareLink continues us on our journey to an electronic health record
As the various components of CareLink are implemented, the patient’s health history and medical information will be linked into a comprehensive patient record, readily accessible and available electronically. In turn, we can coordinate care, reduce paperwork, increase efficiency of service and improve the overall health care experience.

By incorporating quality and patient safety, evidence-based care, comprehensive care coordination and operational redesign, we will accomplish this process of clinical transformation.
