Implementation
Located in the Mary Abbie Berg Senior Center in Mobile, AL, Mercy Medical’s Care Resource Center provides a variety of services to senior citizens in Mobile and Baldwin Counties, in cooperation with Senior Citizens Services, Inc. Information and referral is provided by telephone, and follow up is conducted to ensure that needed services are received. In-take admissions, social services, nursing, and C.N.A. services are provided to the Adult Day Health Care program through the Care Resource Center.

The Community Care and Wellness Program provides home- or office-based assessments of a senior citizen’s physical, emotional, social, family support, and safety status by a Registered Nurse. A Care Plan is developed with client and caregiver participation to the physician and/or community resource to access services for the client. The nurse also provides health education and screening for the client, for Senior Center participants, and for selected Senior Housing facilities.

Other services through the Care Resource Center include advocacy for new services in the community; networking with community agencies, churches, civic organizations, and senior citizen groups; and provision of direct emergency and crisis assistance.
