Description of the Best Practice
SUB-ACUTE CARE

Implementation
Sub-acute care at Mercy Medical is unique in that the patients receive an acute care level of nursing that focuses on rehabilitation. These patients have many complicated, Interacting diagnoses. The following serve as examples of special care procedures:

1. Ventilator Management - management of the vent, weaning the patient from the vent, and patient/family teaching for home management.

2. IV Therapy - management of central lines, the administration of blood products, hyperalimentation, and long-term IV antibiotic therapy.

3. Tracheostomy Management - weaning and home management education for patient/family.

4. Dialysis Management - monitoring for complications (e.g., CHF), monitoring diet/fluid restriction compliance.

5. Medical Management - for complex medical problems requiring diagnostic follow up and post acute care treatment. During the time the patient is receiving these special care needs, a rehabilitative program is woven into place utilizing the services of physical, occupational, speech, and respiratory therapies as indicated.
