Implementation
LIFE Pittsburgh (Living Independence for the Elderly) is the first Western Pennsylvania PACE program. PACE is a type of integrated delivery network (IDN). The program is currently serving over 3,500 enrollees at 12 sites nationwide (ranging from hospitals to nursing homes to stand-alone community providers). PACE has two key elements: (1) an interdisciplinary care team led by a staff physician and composed of both medical and social practitioners; and (2) a day health center. Enrollees visit the center approximately three times per week for intensive medical services, rehabilitation services, or social activities. The principals of PACE are all-inclusive care, integration of service, capitation of finances, and provider assumption of risk.
