Description of the Best Practice
CONTINUUM OF CARE FOR VENTILATOR PATIENTS

Implementation
The ventilator programs as Mercy Medical were developed to meet the needs of ventilator dependent patients. With the goal of maximum independence, patients are screened prior to admission for proper program placement. Most patients begin active weaning within 10 to 14 days of admission. Care plans for each patient are developed with input from physicians, nursing staff, dietitians, pastoral care, and respiratory care staff. Patients are participants in the decision-making process as the treatment plan is implemented. Mercy medical is the only local facility that uses portable ventilators, allowing patients to move within the facility for treatment in physical and occupational therapy settings. Patient education is an integral part of the program in order to achieve a balance of meeting the physical, spiritual, emotional, and medical needs of the patients.
