| Not submitted via form A comprehensive array of supportive services is provided at The Imperial on Peachtree, The Edgewood, Welcome House and Santa Fe Villas to a diverse population of low, and very low income formerly homeless adult consumers with special needs; i.e., with mental illness, HIV/AIDS, and/or substance dependence diagnoses. The goal of the supportive services program is to assist consumers to be successful living independently in the community. This goal is implemented by providing opportunities for consumers to gain access to the necessary resources for them to achieve their individualized and community goals with the support of a network of community service providers and services coordinated and complemented by three Mercy Mobile Health Care staff teams in the following focus areas: goals development and counseling, community development, and education and training. Mercy Mobile Health Cares role in providing supportive services began at The Edgewood in 1995; at The Imperial in 1996; at Welcome House in 1998; and at Santa Fe Villas in 1999.
